Registered Nurse (RN)
JOB SUMMARY:
A Registered Nurse (RN) administers accomplished nursing affliction to patients on an alternate base in their abode of residence. This is performed in accordance with physician orders and plan of affliction beneath the administration and administration of the Director of Clinical Services/Nursing Supervisor.
QUALIFICATIONS:
1. Graduate of an accustomed academy of able nursing and currently accountant in the state(s) in which practicing.
2. Two (2) years nursing experience, preferred.
3. Acceptance of aesthetics and goals of this Agency.
4. Ability to exercise action and absolute judgment.
RESPONSIBILITES:
1. Provides casework in accordance with the plan of care.
2. Makes the antecedent appraisal appointment and consistently reevaluates the patient’s nursing needs.
3. Initiates the plan of affliction and all-important revisions.
4. Provides those casework acute abundant specialized nursing skills.
5. Initiates adapted antitoxin and antidotal nursing procedures.
6. Prepares analytic and advance addendum for anniversary accommodating and appointment summaries of affliction conferences on his/her patients in a appropriate address per Agency policy.
7. Coordinates services
8. Informs cadre of changes in the action and needs of the patient.
9. Counsels the accommodating and family/significant others in affair nursing and accompanying needs.
10. Participates in and presents in-service programs
11. Understands and adheres to accustomed Agency behavior and procedures.
12. Processes orders and notifies physician of accommodating needs and changes in condition. Completes certification/recertification orders and acquittal summaries.
13. Determines the bulk and type of nursing bare by anniversary alone patient.
14. Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker those patients acute their specialized skills.
15. Supervises and teaches added nursing personnel.
16. Conducts accommodating affliction conferences on patients assigned to his/her care.
17. Participates in associate analysis and achievement advance as assigned.
18. Participates in appliance analysis of medical annal as assigned.
19. Gives absolute accommodating affliction as needed.
20. Takes on-call assignment nights, weekends, and holidays, as assigned.
21. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and cogent change in action in accordance with Agency authentic time frames.
22. Appropriately utilizes ICD-9 codes.
WORKING ENVIROMENT:
Works central in Agency appointment and patients homes and campaign to/from accommodating homes.

1. Supervised by: Director of Clinical Services/Assistant Director of Clinical Services/Team Leader
2. Workers Supervised: Licensed Practical Nurse, Home Health Aide, & Homemaker
